# Reading Salisbury, Part 1: The Old Wisdom We Threw Away

I’m 57 years old. I have spent a good part of those years being wrong about things that mattered, and I have the scars, the receipts, and the old photographs to prove it. This blog is called Grizzled Wisdom because that is the only kind I have. I didn’t earn it in a classroom. I earned it by doing the wrong thing for a long time, paying for it, and eventually noticing.

Two subjects are going to come up here more than any others: what we eat, and what we do with our money. I’ve made a mess of both. I’ve also cleaned up both, late in the game, and the thing that surprised me most in cleaning them up was how little of what actually worked was new. The rules that fixed my body and the rules that fixed my finances were, almost without exception, rules that somebody’s grandfather already knew. We didn’t discover them. We forgot them, and then we spent decades and a great deal of money being told the forgetting was progress.

That is the thesis of this blog, if it has one. Most of what I’ll write here is old. Some of it is very old. My job isn’t to invent anything; it’s to dig up what got buried, brush it off, and explain why it still works, using my own mistakes as the illustrations. If you’re younger than me, I’d like to save you some of the years. If you’re my age or older, I’d like to convince you it isn’t too late, because it wasn’t for me.

So for the first real posts on this site, I’m not going to tell you what I think. I’m going to read you something a doctor wrote in 1888, and we’re going to figure out together how it got lost.

## The book

The book is “The Relation of Alimentation and Disease,” published in New York in 1888 by James Henry Salisbury. “Alimentation” is an old word for feeding, for what you take in. The title, in plain English, is “How What You Eat Relates to What Makes You Sick.” Salisbury wrote it for other doctors, after thirty-some years of experiments, and he waited more than twenty of those years before publishing, because he wanted enough cured patients on the record that nobody could wave him off.

If the name Salisbury sounds familiar, it should. He is the Salisbury of Salisbury steak. His name ended up on a school-lunch tray and a frozen TV dinner, a slab of mystery meat under brown gravy, and I think that is one of the saddest little jokes in the history of American food. The man spent his life arguing that lean beef, properly prepared, was medicine, and that most of what we call disease begins at the table. A century later we honored him by turning his name into the cheapest thing in the freezer aisle. That is the forgetting, in one dish.

I found a scan of the original book, digitized from a British university’s medical library, and I’ve been reading it the way you might read letters from a great-grandfather you never met. It is long-winded in places, confidently wrong in others, and every so often it says something so plainly that I put it down and stare at the wall for a while. I think it deserves a slow read, and I’d rather do that in public, with you, than alone.

Here is how this series will work. Each post will cover a short stretch of the book, sometimes a single paragraph, sometimes a page or two. I’ll quote him directly. I’ll tell you what I think, what I’ve seen in my own body, and where I think he’s off. I’ll try hard not to turn a nineteenth-century physician into a prophet just because he happens to agree with me about meat. He doesn’t need my help to be interesting, and he doesn’t need me to pretend he was right about everything.

But before we open the book, I owe you the rest of the story: what happened to this wisdom, and how it wound up in my hands.

## How we lost it

Salisbury wasn’t alone. That is the first thing I want you to understand. In the second half of the nineteenth century, the idea that meat was the natural food of a healthy human, and that starch and sugar were what made people fat and sick, was not a fringe position. It was one of the serious positions.

In 1863, a London undertaker named William Banting published a little pamphlet called “Letter on Corpulence,” describing how he had lost a great deal of weight by giving up bread, sugar, beer, and potatoes and eating meat instead. It sold enormously. For decades afterward, “banting” was simply the English word for dieting. Salisbury, working in New York in the same years, was arriving at a stricter version of the same conclusion by a very different road, and by the 1880s his method was in use by physicians in England as well as America. He says so himself in the Preface, and there were English patients writing whole books about what his beef-and-hot-water regimen had done for them.

Then, over the next sixty or seventy years, the whole idea went away. Not because it was disproven. Because other things happened.

The germ theory of disease won, and it deserved to win; it saved more lives than anything before or since. But its victory changed what doctors were looking for. If disease came from a microbe, then the interesting questions were about microbes, and the old doctors who kept asking what the patient had been eating started to look quaint. Salisbury himself had a foot in both camps, as we’ll see, and I think that is part of why his book slipped through the cracks.

Meanwhile the food itself changed. Industrial seed oils appeared in American kitchens in the first decade of the twentieth century and were marketed as cleaner and more modern than lard and butter. Cheap sugar and refined flour went from luxuries to staples. The breakfast-cereal men in Michigan built an empire on the moral idea that meat was coarse and grain was pure. And in the middle of the century, a persuasive scientist named Ancel Keys convinced the medical establishment that dietary fat, especially animal fat, was what clogged the arteries of American men. Whether he was right is a fight for another post. What matters here is that by 1977, the United States government was officially telling its citizens to eat less meat, less fat, and more grain, and by 1992 there was a pyramid on the side of every cereal box with bread at the bottom and beef up near the top, next to the candy.

I was eight years old when the government published those first dietary goals. I was twenty-three when the pyramid came out. I did exactly what it said. So did my parents, and so did my doctors. And what happened to me over the next thirty years is what happened to most of the country: I got heavier, then heavier still, and every time I went in for a checkup there was a new number that was a little worse and a new prescription to address it.

This is the part I need you to hear, because it is the whole point of the blog. Nobody lied to me. The people who wrote the pyramid believed it. My doctors believed it. I believed it. We had simply, collectively, forgotten something that a doctor in New York had figured out by feeding beans to six hired men in 1856, and there was no one left alive who remembered.

## How I found it again

I’m not going to tell my whole story in this post; there will be plenty of room for that. The short version is that somewhere in my fifties I ran out of road. The weight, the numbers, the energy, the way I felt when I got out of a chair. I had tried the sensible things, the portion control and the whole grains and the lean chicken breasts, and they had worked about as well as they work for everyone, which is to say briefly.

What finally worked was the least sensible thing on the list. I stopped eating plants. All of them. I ate meat, and eggs, and I drank water, and I waited to see what would happen. What happened was that within a few weeks I felt better than I had in years, and within a couple of years I was a different man.

The thing I want to emphasize is that I did not read Salisbury first and then eat this way. I ate this way, got well, and then went looking for the history, half expecting to find that I had stumbled onto something new. Instead I found Banting in 1863, and Salisbury in 1888, and an Arctic explorer named Vilhjalmur Stefansson who lived on nothing but meat for a year under medical supervision in a New York hospital in 1928 and came out healthier than he went in. I found a whole lineage of people who knew this, wrote it down, and were forgotten.

That was the moment Grizzled Wisdom became a blog rather than a private notebook. Because if the diet that saved me was old, I started to wonder what else was. And the answer, when I looked at my finances, turned out to be: pretty much everything. The rules that dug me out of debt were the rules my grandfather lived by and I had decided were old-fashioned. Spend less than you make. Owe nobody. Buy the thing once, and buy it well. We’ll get to that. For now, just notice that it is the same story: old wisdom, lost, rediscovered by someone who first had to make the mistakes.

Now let’s open the book.

## Who was this guy?

Salisbury lays out his credentials in the first few paragraphs of the Preface, and they aren’t nothing. Before he was a doctor, he worked in what he calls “the exact sciences of chemistry, botany, geology, zoology and mineralogy.” In 1846 he was made assistant in the Chemical Laboratory of the New York State Geological Survey, and by 1849 he was running it. He graduated from Albany Medical College and entered practice in 1850.

That background matters for how he thinks. He is not a bedside physician who picked up a microscope as a hobby. He is a chemist and a microscopist who became a doctor, and he brought the laboratory habit with him: look at the thing, draw the thing, measure the thing, and don’t trust the textbook until you’ve checked.

I want to say a word here about credentials generally, because it comes up. A lot of the people who rediscovered this way of eating in our own time were not doctors. Some were, and some of the doctors have paid a professional price for it. But the thing that makes Salisbury worth listening to is not the letters after his name, of which he had plenty and lists them all on the title page. It is that he ran the experiment. He is going to tell us exactly what he did, on whom, and what happened. That is what I trust, and it is why I trust my own experience over a pyramid.

## “The art of Therapeutics was a chaos”

What strikes me most in the opening pages is how frustrated the young Dr. Salisbury was. He describes being “immediately and forcibly struck by the almost entire want of medical knowledge in regard to the true Causes of Disease.” The medicine of his day, he says, treated “established pathological conditions as though they were the disease itself, rather than what they actually were,” which is to say “consequences based upon antecedent and obscure states arising from an unknown Cause.”

Read that again slowly, because it is the whole book in one sentence. The symptom is not the disease. The tumor, the cough, the sugar in the urine: those are downstream. Something upstream is causing them, and if you only ever treat downstream, you will be busy forever and cure nothing.

If you have ever sat in an exam room and been handed a new prescription for a new number, while nobody asked what you had for breakfast, you know exactly what he is describing. He was describing it in 1888\. It has not gotten better; if anything the machinery for treating consequences has gotten so good that it is easier than ever to never ask about causes.

He then makes a promise to himself that I find moving. Looking at the “grim list of so-called ‘incurable diseases,’ and their steadily increasing death rates,” he decides that they “must be curable,” that “their causation must be discoverable,” and that he will find it, in his words, “before my exit from this world.” He says he “started in without theories, without prejudices,” with “no beaten rut to confine me,” resolved “to collect and sift actual facts.”

Now, no one starts without prejudices. He had plenty, and we’ll bump into them. But the intention is the right one, and it is more than most of us manage. It is certainly more than I managed for the first fifty years, when my entire method for evaluating dietary advice was whether it came from someone in a white coat.

## Five years of drawing everything

Here is where the chemist shows up. Salisbury describes spending the early 1850s making “most searching examinations of every element of the human body, in health and in disease, both microscopically and chemically,” and making “detailed drawings of every form met with.” Blood, stool, urine, sweat, ear wax, the contents of hair follicles. Everything. He filed it all away, hoping that eventually each disease would “furnish its own history.”

And it didn’t work. He is refreshingly honest about that. He calls those first five years “very laborious, anxious and painful ones,” and says the work went on “seemingly without resulting in much that was of real utility.” He was looking at effects, and effects don’t tell you their cause just because you stare at them long enough.

There is a grizzled lesson buried in that paragraph, and it applies well beyond medicine. Data is not understanding. I have spent years tracking things, weight and calories and account balances, with great diligence and no result, because tracking the consequence of a behavior is not the same as changing the behavior. Salisbury had five years of beautiful drawings and no cures. What he needed was not more drawings. It was an experiment.

## The experiment that changed everything

Then comes the paragraph that made me want to write this series.

“In 1854 the idea came to me, in one of my solitary hours, to try the effects of living exclusively upon one food at a time. This experiment I began upon myself alone at first.”

One food at a time. On himself. Starting with what the physiology textbooks of his day ranked as the most nutritious food available, which was, I am not making this up, baked beans.

It took three days. He reports becoming “very flatulent and constipated, head dizzy, ears ringing, limbs prickly, and was wholly unfitted for mental work.” Under the microscope he found the beans had not digested at all; they had fermented, filling his gut with “yeast, carbon dioxide, alcohol and acetic acid.” The starch, he concluded, was locked inside sacs his digestive fluids couldn’t open, and by the time fermentation blew them apart, the food had been “too far changed into gas, alcohol and vinegar, to afford much nourishment to the body.”

I want to be careful here. Some of his chemistry is off, and we’ll get into that as we go. But set the mechanism aside and look at the method. In 1854, a working physician decided that the way to find out what a food does to a body was to eat only that food and watch. That is an elimination diet. That is what people now call n=1 self-experimentation, as if it were a new idea. It is, more or less, what every one of us did the first week we went carnivore and noticed that something we had assumed was just part of getting older had quietly stopped.

He says of that first bean experiment: “I had found a door standing ajar, through which I began to get glimmerings of light in the right direction.”

That line is why I keep reading him. Not because he was right about everything, but because of that door. He noticed the door. Most people, most doctors, most of the nutrition establishment for the last seventy years, walked right past it.

## Then he hired volunteers. Then he bought two thousand hogs.

From 1854 to 1856 he kept running single-food trials on himself, “as often as I could find time to do so,” which I take to mean that he had a practice to run and did this on the side, like a man with a very strange hobby. Then, in September 1856, he “hired six well and hearty men” to live with him on baked beans. In 1857, four more men lived on nothing but oatmeal porridge for thirty days. And in 1858, because you cannot run an experiment on humans “to the death point,” he took “nearly 2,000 hogs in separate lots and in different pens” and tested different feeding regimens on them, tending, feeding, and dissecting them himself. His comment on that work is my favorite line in the Preface: “it was not work that could be done with kid gloves on!”

Sit with that for a second. Two thousand hogs. Personally. A physician with a practice in the 1850s did his own animal-feeding trials at a scale that would require a grant committee and a university today, because he wanted to know, and nobody else was going to find out for him.

By 1858, he says, he had begun “to understand from what Cause all our diseases emanate, excepting those arising from injuries, poisons and infections.”

That is an enormous claim, and I don’t think it holds up as stated. But notice the shape of it, because it is exactly the shape of the argument we are still having today: apart from the things that break you from the outside, most chronic disease comes from what you put in your mouth. He then spent another twenty years, by his own account, holding the book back “in order that sufficient cures of so-called ‘incurable’ maladies might place both discovery and method of treatment beyond all reasonable doubt.” He says that “hundreds of cures now attest to their utility, not alone in my own practice, but also in that of other physicians of high repute, both here and in England.”

Twenty years. He sat on the thing for twenty years because he wanted the results to speak before he did. I don’t know many people, in medicine or anywhere else, with that kind of patience. I certainly didn’t have it. Half my mistakes came from acting on a conclusion before I had checked it.

## What he got wrong, so you know I’m not selling anything

I’ll say the obvious thing out loud: this book is 138 years old. Salisbury believed things about germs, and about what he called “vegetations” growing in the blood and tissues, that did not survive the next few decades of microbiology. He was writing six years after Robert Koch identified the tuberculosis bacillus, and you can feel him, in the Preface, trying to hold his own germ theory and his dietary theory in the same hand. He spent, by his account, fifteen years on smallpox and years more on measles, malaria, and diphtheria, and much of what he concluded there is simply not how those diseases work.

Anyone who tells you Salisbury was right about everything hasn’t read him. Anyone who tells you that, because he was wrong about smallpox, he must have been wrong about beans, hasn’t thought about it very hard. Old wisdom comes to us tangled up with old error. Part of the work of rediscovery is separating the two, and I’d rather do that carefully, in the open, than pretend the errors aren’t there.

But here is what I keep coming back to. He looked at sick people and asked what they were eating. He tested foods one at a time, on himself first. He noticed that starches and sugars fermented in the gut and made people miserable, and that lean meat did not. He built a treatment around broiled beef and hot water and got results good enough that physicians on both sides of the Atlantic adopted it. And he did all of this without a single one of the tools we lean on now: no glucose monitor, no lipid panel, no double-blind trial, no internet forum full of people comparing notes. He had a microscope, a notebook, some very patient hired men, and a great many hogs.

## The dedication

He closes the Preface by dedicating the book to “the medical graduate of to-day,” in the hope that “the solicitude which clouded the outset of my career may be removed from theirs.” He wanted the next generation of doctors to not feel as lost as he had felt in 1850, staring at a list of incurable diseases with no idea where any of them came from.

I think he would be disappointed by how that turned out. The medical graduate of today is, if anything, more certain than Salisbury’s contemporaries were, and certain of nearly the opposite thing. But I also think there is something in these pages for those of us who found our way back to meat by trial and error, one food at a time, in our own solitary hours. We are, whether we know it or not, his students. This series is my attempt to know it.
